Job Summary
The LaST supports staff in understanding and responding to individual student needs and implementing adjustments in a precise and personalised manner, ensuring students can access learning on the same basis as their peers.
Duties:
- Attend and action recommendations in LST and Complex Case Meetings.
- Attend student meetings at the request of HTW, HTCL or DPs.
· Provide direct support for the implementation of targeted intervention as directed by LST or individualised student plans.
· Complete Student Learning Profiles for new students.
· Complete Sentral Reviews or File Reviews at the request of DPs.
· Develop, implement, monitor, evaluate and adjust individualised student plans in consultation with student, family, external/DoE services and staff.
· Complete applications for Assisted School Transport Program for mainstream students.
· Build capacity in classroom teachers to understand and incorporate instructional, environmental and assessment accommodations into their practice.
· Conduct functional behaviour assessments and classroom observations to inform adjustments and review individualised student plans.
· Upload student documentation regarding learning and wellbeing needs to SharePoint > Care Continuum > Student Folders
· Complete Informal Advice, Access Requests and Request for additional funding (IFS).
· Complete Annual IFS reviews in Term 3 and IEP reviews each term.
· Maintains the Disability Provisions list and supports staff to implement provisions.
